Why should I avoid facial massage after jaw botox?

The Botox may spread to areas like other muscles that you don't want to be affected. It may lead to side effects like an asymmetrical smile. Now the good thing is that usually Botox is absorbed by the muscle within a few days to a week. And then after, no matter how much you massage simply leaving it alone, it makes no difference. So you don't have to avoid facial massage forever after your Botox procedure. Just for maybe the first week or so. After that, the Botox will be absorbed or bound onto the receptors on the muscle and it's not going to migrate any further.

How can thread lift be removed?

There are special hooks that you can use to pull out one end of the thread and just pull the hooks to retrieve the thread. If you want to redo or replace the threads, I usually recommend to wait for a week or so, before replacement of new threads. This is done under local anaesthesia.

How effective is scarless eye bag removal, and how is it performed?

If the eyebags are of a certain size and not too big, scarless eyebag removal is appropriate. The problem with larger eyebags is that after removal, you end up with a lot of loose skin. So the problem is how to address this loose skin. We also address this in this Q&A here, which discusses skin tightening. If it's not too large an eyebag, we use noninvasive skin tightening with energy-based devices. But on the other hand if its big enough, like the large eyebags with very loose skin, skin excision is often required.

Why do women have a higher risk of developing carpal tunnel syndrome?

Statistically speaking, carpal tunnel syndrome in women is more common. Why this is the case could be due to a variety of reasons. Firstly, women have smaller wrists. So, they have less space in the carpal tunnel to begin with. There's less tolerance for swelling inside the tunnel. Smaller wrists, smaller tunnel, and then less tolerance for increased volume inside the tunnel. So, if the tendons swell from computer work, obviously there's less space and these tendons press on the nerve. Secondly, women are often doing a lot of manual work, which would precipitate carpal tunnel.

As a teenager with eyebags and dark eye circles, what are my most effective treatment options? (photo)

Safety is important. The skin quality in younger patients is usually quite good. With minimally invasive treatments for eyebags, if the eyebag needs to be shrunk, we use AGNES. AGNES is safe. Next, for tear troughs and deep hollows, we have the option of fillers which are often very useful. And very safe. For dark eye circles, sometimes it's the hollowness; if there is pigmentation underneath the eyes, a combination of radio-frequency or lasers can sometimes be useful.

How long will it take for face asymmetry caused by botox to wear off?

We tell patients that the Botox procedure needs to be repeated every three to four months. Although we tell patients it will last for four to five months, sometimes six months, that really depends. When you do get a side effect like an asymmetrical smile, it can be adjusted by adding a little bit more Botox to balance out the antagonistic muscles. The effect of Botox on the unevenness usually wears out in about four to six weeks anyway. It doesn't really last three to four months.

How likely will botox injected into the top lip and chin cause bone loss in the jaw?

The answer is not likely. It’s certain that Botox is a topic that has no relevance to the jaw, meaning the chin or mandible. There’s no effect on the mandible. The mandible tends to shrink due to atrophy. So as you get older, the mandible shrinks. Also, you tend to lose teeth and you lose other bone support. This shrinking of the mandible has been proven by CT scans. We know that the facial skeleton changes with age, it tends to get smaller and the mandible tends to lose its mass. It is unlikely that this is caused by Botox; it is just a natural ageing process.

How effective is cosmetic lower eyelid blepharoplasty, either the transconjunctival or infraciliary approach, in permanently removing eye bags? (photo)

Very effective. A transconjunctival blepharoplasty typically means that the removal of, or reduction of the three pockets of fat under the eyelid through an incision, a hidden incision that is inside the eyelid. So you see when I turn down the islet, you can see the red mucosa. We go in through there. It's scarless—leaving no visible scars. And we can help to remove the fat. It's good in that sense, but it cannot shrink the skin. If you’ve got a big eyebag and you’re going to remove the fat, then the skin is loose. It cannot address loose skin on its own.

From the photo, the small raised bumps appear to be 'vesicles' or very small, fluid-filled blisters. These vesicles may occur on the fingers, palms and occasionally on the soles of the feet, and are due to a condition called Dyshidrosis (or dyshidrotic eczema or pompholyx eczema). The blisters are usually itchy and may be filled with fluid. Blisters normally last for about two to four weeks. Given your short account of the problem, it would appear that your stiffness is caused by a tendon inflammation (tenosynovitis) - swelling and irritation of the flexor tendons that bend the fingers. At one end of the spectrum (in the acute or active phases) there is pain, swelling and stiffness of the fingers, while on the other end of the spectrum 'Triggering', locking or clicking occurs as the inflamed tendons have difficult passing through tunnel sheaths (called pulleys) that keep them in place. The pain is most tender in the palm around the first knuckle.

Will my numbness go away after ulnar nerve transposition surgery?

I'm sorry to hear that you are still symptomatic after your surgery. It is certainly unusual for you to have symptoms of cubital tunnel syndrome in both hands at the same time. As you are already seeing both a neurologist and a hand surgeon, I am sure a variety of tests were already performed to establish your diagnosis before surgery. There are various types of surgery for cubital tunnel syndrome (ulnar nerve impingement at the cubital tunnel of the elbow).
