From your description, you sound like you may have comedonal acne. We can investigate if there is a hormonal component to it. I would be interested to know how you responded to your previous treatment with Accutane.

I imagine you would have had a good response but it flared when you stopped the treatment. If that is the case, we can explore reasons for the flare e.g. treatment was too short, dose was suboptimal.

From my experience, comedonal acne responds to medicines in the Accutane family very well. Most people will experience an excellent improvement during treatment, and remain well for months to years even after treatment is stopped, provided the right dose and right length of treatment is given.

Sometimes other treatments such as extraction and lasers can be performed in conjunction with low dose oral isotretinoin for optimal results.

I would advise you to seek a consultation with a trusted doctor or dermatologist if it continues to bother you.

You can consider chemical peels as an addition to the retinoids. This will help with 'releasing' the comedones without requiring painful extractions. Chemical peels, when performed by doctors, are very safe.

It is very common for ladies to have clogged pores in the lower face, especially near the time of the month. A regular exfoliation and oil control regime should be sufficient to control what you described. Differin applied properly (for example, only at night) should have good effects, if not you can also consider an increased concentration. Additionally, there are some other skincare products like BHAs you can use in the day.

Additionally, you may consider seeing a doctor to check if you have diagnosed your own condition accurately. Some other types of facial growths could mimic stubborn whiteheads. Your doctor may also suggest some medical treatments with medi-facials, chemical peels or lasers to improve your condition.

If you are a male patient, I would opt to do Accutane and then tail down the dose and keep on a very low maintenance dose (which virtually eliminates side effects) and yet keeps the oiliness and pimples at bay. Topical products actually play a huge role in skin care. I have seen so many patients who come completely clueless, and using the most oily Korean moisturisers for their already oily skin--- because Korean stars are using it.

What is the best acne treatment for recurrent breakouts at temple, jawline and chin?

I noticed that I tend to breakouts after holidays. Yes, weather and humidity changes can sometimes trigger breakouts. Stress/poor quality of sleep due to jet lag and time differences can also contribute. Speaking from personal experience here; I tend to break out much worse in Singapore than I did living in the UK. Went to national skin centre for consultation, and was given some watery creams and Cetaphil products to use. Hard to comment on what you were given, but from what you’ve described here, NSC doesn’t seem to have given you anything specifically for acne treatment.
