What alternative treatments are there for an ankle injury if physiotherapy doesn't work (photo)?

If you have tried physiotherapy and the ankle remains persistently painful and swollen, it means that it was not a "simple ankle sprain". We need to do a thorough investigation to find out what structure has been injured. An MRI would be recommended as it could visualize the soft tissue better. It could be hairline fracture, a ligament tear or an osteochondral injury (a small part of the bone/ cartilage has been chipped off). Once we have established the diagnosis, then we can start the appropriate treatment.

How do we know we have to do ACL reconstruction for an ACL injury?

ACL doesn't heal by itself if clinically unstable. We can test the knee, do the same manoeuvre if the knee is moving like previously, suggest reconstruction surgery, because ligament don't heal itself even if you wait 5-10 years. If you don't mind wearing knee guards, and can just stroll—you might not need surgery. However, patients must be compliant: if you forget to wear it will cause further damage. The problem is the ligament itself, once it stretches, on MRI we still can see one piece of tissue once it loses tension.

Could a burning sensation in my left knee and walking issues be caused by inflammation due to a new injury?

A quick blood test can be for gout, which comes and goes. Maybe there is an existing problem. Keep a journal on these kinds of pain, the timing when it happens, and what you are doing at that time, every time you do something to trigger the pain. If it keeps coming back then we can do a quick blood test.

Post-Total knee replacement, how to work on gait, is it alignment issue?

Knee pain does not occur after one day. Some people learn the wrong gait and feel like the wrong gait is normal. After surgery, they must learn the correct ways. Therapists will come in to teach them to walk properly and re-strengthen the muscle. When I see these patients walk in my clinic normally, I will sometimes not know they have done a TKR. The whole purpose of knee replacement is to restore the mechanics of walking and quality of life.

What will happen after 15-20 years post-knee replacement?

This is a conservative estimation, after 15-20 years or even up to 30 years, it can be working perfectly. Some might have no pain or no symptoms and are absolutely healthy. If unfortunately the knee replacement reaches the end of its lifespan, you can change it. Americans are more advanced, they do a lot more than us in learning how to tackle these problems, thus I learn all their special techniques to do revision surgery. If after 15-20 years you reach the end of the expected lifespan, you can still change it.

How does Japanese turmeric differ from other turmerics for joint pain?

In general, Turmeric contains curcumin, a natural Polyphenol. Curcumin is currently a hot topic in the research community because it has multiple health benefits, such as having inflammatory properties, helping the kidneys function, and improving metabolism. It is highly likely that Curcumin is the reason why Turmeric has been used as a type of medicine. Japanese turmeric is a bit of a misnomer. On the one hand, there are several types of turmeric found in Japan, including wild turmeric (Kyou-ou), Okinawa Ougon Turmeric (Ukon), Spring Turmeric, and Black Turmeric (Gashutsu).

Can Turmeron replace my pain medications that don't seem to work?

No. Unless approved by a doctor, an over-the-counter supplement cannot replace medication prescribed by a doctor. If your medication is not working then you will need to see your doctor to replace or find something to assist your medication. That said, there is no harm in bringing up Turmeron and what it claims to do to your doctor. If there are no potential complications, there are no interactions with your medication, and your doctor clears you to take it, then you can take Turmeron.

Can I just add more turmeric to my meals to stop the pain?

Curcumin is the component of turmeric that has been found to reduce joint pain. Curcumin only makes up around 3% of turmeric used in cooking, meaning every 100mg used will only give you 3mg of curcumin. In contrast, curcumin supplements have curcumin that make up 95% of the dose, meaning you will get 95mg of curcumin for every 100mg of supplements taken. However, there may be unknown positive benefits for taking turmeric as a spice instead of just curcumin as a supplement. If you just want to deal with joint pain, you might need more than just turmeric or curcumin to treat or manage it.

Is there a dose limit to turmeric for joint pain for a 60+ year old person?

While more research is needed to find the recommended dosage of turmeric, randomised control trials (RCT) have used a similar dosage to one another. This meta-analysis found that 8 RCTs used around 1g/day when given without painkillers and 500mg/day when given alongside painkillers. The highest dosage given was around 2g/day for joint pain. See your doctor to get an approximate dosage for your joint pain. There are other turmeric extracts that contain Labdane type Diterpene, such as in Japanese White Turmeric.

How is a SLAP tear in the shoulder diagnosed?

Hi, really sorry to hear about your shoulder injury. SLAP tear means "superior labral tear from anterior to posterior". Labrum is the fibrocartilaginous rim of soft tissue attached around the margin of the shoulder joint (the glenoid cavity). The biceps tendon is also anchored in the superior labrum as well. So the symptoms a SLAP tear are usually pain when the biceps are required, or overhead throwing movement. The pain is especially worse at night, and the patient typically cannot sleep on the affected shoulder.
