The arrow in the picture points to the base of the 2nd metacarpal or 2nd carpometacarpal joint on the dorsum of the left hand. At this location, the most common causes of a painful lump or bump are:

  1. 2nd metacarpal bossing
  2. Synovitis of the insertion of the extensor carpi radialis longus tendon
  3. Synovitis of the extensor indicis communis or propius tendon to the index finger

A hand & wrist specialist will be able to clinch the diagnosis with a detailed clinical examination and imaging modalities such as X-rays, ultrasound or MRI scans. Early treatment will prevent the problem from becoming a chronic one, which may be trickier to treat.

Lumps can be from different causes:

1. Skin

2. Soft tissue

3. Bones

4. Joints

Common causes around the wrist are ganglions and these can cause pain at times. It maybe worth visiting a MSK/Sports Physician for an assessment and sameday ultrasound assessment before deciding on further treatment.

Unfortunately, the swelling is not very obvious from the clinical photograph. I think for a proper diagnosis, you should get this checked out by your doctor.

Some comon causes of swelling around the wrist include a ganglion. But you will require a close examination to make the diagnosis.

What could be the cause of a hard bump on my left thumb joint?

This swelling over the dorsum of the interphalangeal joint (IPJ) of the thumb appears to be a mucous cyst. A mucous cyst is a fluid-filled balloon arising from the IPJ of the thumb. It is commonly associated with wear-and-tear, or osteoarthritis, of the IPJ of the thumb. Overuse and increased motion in the IPJ may cause more joint fluid to fill this cyst, which in turn causes an increase in: pain, redness and swelling at this site. Unfortunately, the only effective treatment for a mucous cyst is surgery.
