Lower cheek fat can arise from the tissue under the skin, or from deeper structures such as the buccal fat pad. The buccal fat pad is an extension of a deep fat pad that communicates with the temple region, and can be prominent in the cheek either due to excess, or due to the ageing process as the fascia holding the fat pad becomes lax. Occasionally, laxity of skin can also masquerade as fat in the cheek due to descent of the facial tissues. Very occasionally, cheek fullness can be due to a growth such as a lipoma.
The treatment for each of the above is different, ranging from liposuction for the subcutaneous fat, to intra-oral buccal fat pad removal, to facelift surgery, to excision of lipoma. The best route to take would be to consult with a plastic surgeon who can diagnose the cause of your cheek fat and recommend the most appropriate method of management.
