When is a total abdominal hysterectomy salpingo-oophorectomy surgery recommended?

Although you do not wish to conceive at this moment, this doesn't mean that you need to undergo a hysterectomy unless that is what you truely desire especially if the fibroids are not deemed to be cancerous.

You can still opt to keep you womb and have all the fibroids removed instead and this operation is called a myomectomy.

Preserving your womb however will mean that you will still get your monthly periods and there is a small chance of recurrence of the fibroids. you will also need to have your regular pap smear screening for cervical cancer. a myomectomy however preserves your fertility should you ever change your mind in the future.

Unless your ovarian cysts are cancerous, i really wouldn't recommend that you have your ovaries removed (oophorectomy) as this will mean that you will undergo surgical menopause at the age 41 (ie. 9 years earlier than the average age of menopause).

You may then experience some if not all the short and long term problems associated with "premature menopause" in your case in particular. it may be better to have the ovarian cysts removed (cystectomy) instead, provided the cyst are not cancerous.
