As orthodontic treatment takes time to complete, typically 1.5 to 2 years, it is not uncommon for some patients to have to switch orthodontists due to different life circumstances. These are some medical information that I feel will greatly benefit your new orthodontist:
- Pre-treatment or starting Xrays (Orthopantogram and lateral cephalogram)
- Pre-treatment photos of your teeth and facial photos
- Study models (if available) or photos of study models, some clinics may have a digital version of the study model if they routinely use an intra-oral scanner
- Your orthodontist's treatment plan (e.g if teeth were extracted and which teeth)
- Brand and prescription of the braces used as well as the current set of wires
- If you are currently on clear aligner treatment, the contact of the laboratory that your orthodontist is working with. If it is Invisalign, your new orthodontist can request for a transfer of the ClinCheck plan
