Congestion and sensitive skin is often a difficult combination to treat because part of treatment of congestion involves exfoliation and oil control which in itself drys the skin.

Treatment of clogged pores and blackheads involves exfoliation with:

1. Medical facials done by medical personal who is familiar with treating acne problems and sensitive skin at the same time

2. Skincare from doctors

Yes you should seek a dermatologist because it is important to be able to strike a balance between controling your acne and yet avoiding making your dry and sensitive skin worse. You need to find an expert who is good at doing so.

From your description, it does seem like you have both sensitive skin with prominent blackheads or comedones.

To get rid of blackheads, there are creams like tretinoin and salicylic acid creams that can be precribed. Extraction of blackheads can also be considered. Many of the prescribe creams like tretinoin cream and salicylic acid though can be potentially irritating and may aggravate dry and sensitive skin.

you seem to have two problems. Firstly use appropriate moisturizer for dry sensitive skin like physiogel and anti-inflammatory cream. as for your blackheads, it should be extracted carefully by therapist and given some topical antibiotics. After the dry and sensitive skin condition has improved, can consider a salicylate peel with more moisturizer for sensitive skin.

Mild steroidal creams or non steroidal creams like elidel or protopic can help to settle the eczema. Avoid scratching and rubbing the area as this will further aggravate the skin. Yoy should also use a gentle soap free cleanser when washing the face. If you are using cosmetics around the eyes, you may want to consider stopping this for a while as some products may cause the skin to become more itchy and red.
