The infection happening around the dental implant screw is causing failure in osseointegration (integration between bone and implant screw) and this means the foundation of the Dental Implant is weak. Ossesointegration is one of the most important process in determining the success or failure of Dental Implant procedure.

(Think of building a house. Without a strong foundation, the house cannot stand and it will collapse sooner or later and this applies the same towards Dental Implant procedure)

So to answer your questions accordingly:

Question 1: Is extracting the only option for an infected tooth implant?
Answer 1: In most cases, extracting the dental implant is the only option and with the pictures you have provided, i would agree with your dentist on extracting the Dental Implant. Wait for at least 3 months to allow full gum and bone regeneration before considering another Dental Implant procedure on the same site.
(An estimation for gum regeneration takes about 1 month and bone regeneration takes about 3 months)

Question 2: Otherwise, what are the best options and recommended follow-up procedures? (eg. Root Canal, Bone Grafting, Dentures etc.)
