What options are there in Singapore to check if my ovaries are healthy, especially when they are of different sizes?

I would advise that you see your family physician or gynaecologist for a complete evaluation of your ovarian function.

I would usually start off with a complete medical history review of my patient, with a focus on the menstrual history (when menses started, the regularity of menses, flow, etc), sexual and contraceptive history and any previous tests, treatment or surgeries performed on the female genital organs.

I would then proceed to a physical examination of my patient with a focus on the pelvic area, looking for any signs of abnormalities.

Next, I will proceed to investigations to determine further the health of the ovaries. Laboratory blood tests of certain female hormones like Follicle Stimulating Hormone (FSH), Luteinising Hormone (LH), progesterone, estradiol and Anti-Mullerian Hormone tests may be performed. An Ultrasound of the pelvis to evaluate the female organs will also be required.

Based on the above basic examination and tests, I will then explain the results to my patient and decide on the need for further in-depth testing.

Should I be concerned that my ovaries are of different sizes?

I presume that your gynae performed the ultrasound scan to rule out PCOS (given that you also have irregular periods). You have a cyst in your right ovary - cysts are very common in women who are still ovulating, and are almost always benign (none-worrying, and go away by themselves). This can account for the difference in size between your ovaries. Ovaries are usually NOT of exactly the same size, and range from about 2 - 3 cm x 2 - 3 cm in size. If your ovaries are otherwise normal in appearance (as the report suggests), there is no reason for concern.

How can I reduce the pain of pap smear tests due to dryness and menopause?

Pass urine before the test: Pap smears may cause mild discomfort due to a sensation of pressure in the pelvic area. Urinating beforehand can relieve some of this pressure. In some cases, your doctor might require a urine sample for other testing purposes, thus check with the doctor or staff before going to the restroom. To perform the pap smear, your doctor will use an appropriate amount of lubricating gel to lessen any discomfort. It is also important to take deep breaths and try to relax your pelvic muscles during the procedure.
