How can I get the best results after an angioplasty?

Doctor's Answers 3

I agree with Dr Julian Tan's reply. Angioplasty is NOT a cure. It is a temporising measure to improve symptom and buy patient more time.

1) compliance with medication. Do not stop any medication without consulting your cardiologist.

2) control your risk factors with combination of lifestyle change and medications.

a) cholesterol level is a relative thing; whatever your cholesterol is, it is too high for you and need to be brought down, since you already have blocked artery. While lifestyle measure is important, you will need cholesterol lowering medication to hit target. This is because 2/3 of our cholesterol is produced by our body based on our genes.

b) control BP and diabetes if you have these conditions

c) stop smoking; you may need help to do it. Smoking damages almost every part of your body.

d) lose weight; see a dietitian if necessary

e) increase exercise (>90 minutes/week, preferably >150 minutes/week); the type of exercise regime to undertake depends on individual. You may need help (trainer) to do this. Exercise is not just for your heart but for your overall body well being.

f) lose weight if your BMI > 23

g) lower stress level in life

Despite all the above advice, I always remind my patients to enjoy life. Most things, if done sensibly and in moderation, are ok. Doctors' role is not just to prolong people's life, but to help them to have a happy and productive life. Of course, it would be nice to have both!

I'm glad to hear that you are determined to make healthier lifestyle choices to get the best results after your angioplasty!

I warn all my patients that an angioplasty is NOT a cure for heart disease.

It can certainly help to control your symptoms, but it does NOT fix the underlying heart disease that caused the symptoms in the first place.

To reduce the risk of further heart issues, you'll need to make important changes to your lifestyle.

All my patients get a customised care plan and cardiac rehabilitation programme after their angioplasty.

Getting the best results after your angioplasty reallly depends on your willingness to see the cardiac rehab programme through:

  • Create a plan to stop smoking.
  • Try to lose weight, even if it’s only a few kilos. Even a 3 to 5 kg weight loss can significantly reduce your risk of heart disease.
  • Make exercise a part of your daily routine. Regular physical activity releases hormones such as endorphins and vasodilators that help you maintain healthy blood vessels.
  • Monitor your blood pressure, cholesterol, and blood sugar. Also be sure to limit your sodium intake and eat healthy foods.
  • Take all medications prescribed by your doctor.
  • Visit your doctor regularly.

Finally, you should aim to return to work and all your other activities sooner - this lowers your chances of having further chest pain and anxiety.

When is an angioplasty or angiogram performed in Singapore?

A coronary angiogram is performed whenever a patient has symptoms such as breathlessness or chest pain on exertion (not always) which usually indicate poor blood flow in the heart blood vessel due to blockages. This could a non-invasive angiogram (ie CT coronary angiogram) or an invasive coronary angiogram (gold standard test). The modality depends on the risk and if the risk is higher an Invasive coronary angiogram maybe warranted.

Why do doctors not perform angioplasties for minor blocked heart vessels?

All procedures are carried out after a clinical assessment of risk vs benefit. As mentioned above there is always a risk to any invasive procedure and angioplasties on minor vessels just do not justify a patient undergoing the risk of an coronary angioplasty as some of the risk, though of low frequency, if they do occur can be significantly life altering. And while treating a severe narrowing in an important vessel has a favourable prognostic benefit (ie prolongs life but this is a hidden benefit) the same does not pply to smaller vessels.
