Does the treatment of keloids differ according to the body part where it occurs, for example at the hand versus on the face? (photo)

Hi,

Intralesional steroid injection is the mainstay of keloid treatment. The injectable steroid comes in different strengths.

Facial keloids generally respond better than non-facial keloids to treatment. As such, lower strength steroids are used to inject them and to reduce potential side effects such as skin thinning, loss of pigmentation and formation of blood vessels.

For non-facial keloids e.g those on the hands, higher strength steroid injections may be used cautiously if lower strength steroids fail to give adequate improvement albeit the higher risk of side-effects highlighted above.

For other treatment modalities like pulsed dye laser, fractional CO2 laser; response is also better when treating facial keloids compared to non-facial keloids.

Can facial lasers cause keloids?

That really depends on what sort of lasers/skin treatments you are doing. Ablative lasers, deep chemical peels or skin devices like fractional RF breach the skin layer and pose a risk for keloid formation. This is why I personally have both ablative and non-ablative fractional lasers for scar/rejuvenation/wrinkle treatments. If you are only using non-ablative lasers or laser toning, there is negligible risk. In fact, some non-ablative lasers like the pulsed-dye or long pulsed ND:Yag lasers can be useful for treating keloids!

Can cryotherapy remove keloids on the knees?

Cryotherapy can be used as a treatment for keloids. Usually, a few session spaced about monthly may be used. It is unlikely to completely get rid of the keloid but may help to reduce the size of the keloids. Smaller keloids may respond better to this treatment. Combining cryotherapy with intralesional steroid injection is also a good option and this may be more effective in reducing the size of the keloids.
