Can ptosis be corrected without creating a double eyelid?

Yes, it can, that is my straight answer. Because we can create a hidden fold, now, the tricky part is "can ptosis be corrected without creating a cut or scar? " Yes, it can, but that is really for mild ptosis, where we correct with a procedure that does not involve making a cut on the skin. But for mild ptosis, unfortunately, it might not be insurance or Medisave claimable, but it can be corrected.

Is there any difference in treating older patients as opposed to treating a younger patient for Ptosis?

It starts from the beginning, the older patients have a high chance of having medical problems, so we gotta make sure that they are medically healthy and are not on certain medications or blood thinners, so you want to clear that out. The second thing is older patients tend to have more looser skin and are likely to have to live with this droopy eyelid for a long period of time that plays a role in recovery.

Can I physically feel if I have droopy eyelids?

No. This is not something that you can feel, it’s not a traditional muscle tear or injury. Let’s say you go to a gym or doing sports, you know when you have torn your muscle because it gets very painful, ptosis is not like that, you do not have or feel a sudden acute pain, you do not feel anything snap, it’s not this sudden event but more like a cumulative injury to the tendon. So it may be many years of eye rubbing or many years contact lens wear, things like that, it’s not an acute event.

What are the tell-tale signs for droopy eyelid?

That is a good question. I would say that the number one reason people start noticing or realising something is different is photos. When they look at photos of themselves or when somebody comments on their photos, “Hey, why do you look sleepy? Are you unwell or are you tired? ” and then they realise that they actually feel fine but wonder why do they look tired or why is there an imbalance in their upper eyelids. So that is one common way to tell.

What are the pros and cons of non-incisional and incisional treatments of Ptosis?

For the non-incisional treatments, the obvious pros are that there are no scars and when there is no cut, I would not say that the downtime is shorter, but at least you do not have to wait for the scar to fade. That is the obvious advantage. But I think the cons are a lot more because you are a lot less sure of the amount of tightening you have, a lot of times when we do non-incisional, there is a lot of estimations, so we have to estimate accurately as we can, therefore, advocate this for milder forms of ptosis.

Will there be more types of non-invasive surgery available to correct ptosis in future?

I would say never say never. Because a lot of procedures we do now, if you think of it ten years ago, people will say that’s not really possible, now we have a lot of research, not just for plastic surgery but for any other specialities. Certainly, if we can do something that is minimally invasive, we will choose the more minimally invasive procedure but we also have to be sure that the less invasive procedure produces the same results. There is no point doing a procedure that only lasts 6 months, so that aspect of it is very important.

Will eye surgery such as LASIK or Cataract Surgery cause ptosis?

That’s a good question because a lot of my patients they will have either LASIK or Cataracts done or lens implant around the time of surgery, it does not affect, but the problem really is when they do eye surgery whether its LASIK or cataracts, they need to put a surgical retractor to open the eyeball big for them to do the surgery right, this retraction will worsen any ptosis you have or it may loosen any repair, So I would tell them if they have ptosis repair done, for the first one month, you can't go for LASIK, because you run the risk of having the retractor loosen up the repair,...

What are the main differences between rhinoplasty with cartilage and silicone?

The key differences between using self cartilage and foreign material such as a silicone implant are: Infection risk Minimal with the use of cartilage; approx 1% with the use of silicone implant Extrusion (implant coming out) risk No risk with the use of cartilage, approximately 1-2% with the use of silicone implant depending on technique- cosmetic appearance: cartilage generally gives a more natural softer appearance; silicone implant gives a more defined appearance-cartilage requires a donor site (ear/rib) but silicone implant does not

Before a rhinoplasty, are there any medications that I should avoid?

Firstly, you should always avoid blood-thinning meds. Apart from that, you should also avoid supplements. Here are some examples: Ginkgo Biloba, Ginseng, Cordyceps, Fish oils. Smokers should stop and avoid smoking as it has been reported to give poorer results. Besides that, smokers tend to have a longer healing time as compared to normal patients.

How do I tell if I have gynaecomastia or just chest fat?

Gynaecomastia is different from simply being manboobs; there is a glandular component in addition to a fat component. This breast glandular tissue needs to be removed in order to have a complete correction. If you have gynaecomastia, you should be able to feel a firm breast disc. To determine this definitively, your doctor will suggest for an ultrasound.

How can I reduce the appearance of scars from my previous C-section?

Scars typically appear unsatisfactory in terms of either colour or thickness. In some cases, it could be both. They may also have symptoms such as recurrent itch or pain. Firstly, I have to determine if the scar is a keloid or a simple hypertrophic scar prior to treatment. Next, I will usually start with non-surgical management, with a series of a combination of steroid and botulinum toxin injections. This is applicable only if the scar is amenable. Your doctor will suggest a surgical revision to you only if the scar is overly thick.

How can I elongate my tip after my first rhinoplasty?

Elongation of the tip depends on the direction and degree of elongation required. Often, mild elongation can typically be achieved either non-surgically or minimally invasive surgery. For non-surgically surgery, the doctor may recommend you use fillers or threads. However, it is good to note that these are not permanent. Meanwhile, for minimally invasive surgery, the surgery may require you to use your own cartilage or a silicone implant. This method is really close to the rhinoplasty approach.

What are the different types of breast augmentation procedures in Singapore?

I broadly classify the surgeries I do on the breast into 2 categories - reduction procedures and augmentation procedures. Both also produce a breast "lift" at the same time, with the amount of lift produced differing depending on the type of procedure done. For breast augmentation surgeries, I again separate them into 2 groups - my patients who want implants and those that prefer using their own fat. For augmentation using implants, I use mainly cohesive gel silicone implants that are USFDA approved.

How do breast implants affect breastfeeding?

Breast implants used for breast augmentation these days are most commonly cohesive gel silicone implants. The polymer used is of medical-grade as opposed to industrial-grade silicone, and therefore much more biologically inert and safe. The question I often get asked by my patients with regards to breastfeeding are: Will the silicone leak into the breast milk and make it unsafe for my baby? Will engorgement be worse? Will I be able to breastfeed with the implant in place or will the milk supply be affected? The answer to the first question is a DEFINITE NO.
