What treatment would you recommend for pockmarks and acne scars on the face (image)?

Thank you for the question and the accompanying photos. It seems that you have a combination of a few types of acne scars, with acne pigmentation marks, and may still be suffering from occasional acne. As a first step, I would recommend a 3-5 sessions of subcision to remodel the floor of the deep scars, raise them up, such that they become more level with the surrounding skin. I usually complement the subcision sessions with Rejuran S.

Hi Ai Hoon, Yes, actually, as long as there is a wound, there is a possibility for keloids to form, even on small wounds like laparoscopic wounds. Some risk factors that will increase the likelihood of keloid formation include: 1. wounds forming at high risk sites, like chest, back, upper arms, ear lobes - usually these are sites where the wounds are subjected to high tension 2. genetic predisposition - some individuals are just more prone to develop keloids 3.

Yes, fillers can be done for the upper eyelids. It is especially done for upper eyelid hollowness caused by 2 main issues: 1. age-related volume loss 2. volume loss related to excess peri-orbital fat removal during previous eyelid surgeries Administration of fillers of hyaluronic acid (restylane)or calcium hydroxyapatite has been shown to improve the aged appearance caused by the hollowness. If done right, fillers for upper eyelid hollows is a quick, hassle-free lunch-time procedure.

Sorry to hear that you are still suffering from acne. At your age, you are suffering from this condition known as adult onset acne. Contrary to conventional belief, acne is not a problem faced only during teenage years. Increasing we see adults coming to seek treatments for their acne issues. Adult onset acne tends to afflict females much more than males. And in my practice, I see women between the ages of 30s to 50s coming to see me for acne breakouts. How does applying toner help in skincare?

Hi Tiffany The simple answer is that it depends on your skin. The long answer is that it is more complex than that. That because the term 'toners' has no real fixed definition. Toner, commonly know as a skincare product, is usually used after cleansers. I myself see toner as a second cleanser, as majority of toners has alcohol in its formulations, and work as an astringent. In addition, there are newer formulations of toners out in the market that claim to soothe sensitive skin and even moisturize dry skin.

When is the best time to treat acne scars?

Good to hear that you acne is subsiding! In my opinion, for acne scarring treatments, the best timing to treat scars is as early as possible, when active acne flares are not frequent and severe. I encourage addressing acne scars early due to the 3 main negative factors acting against us: 1. Atrophic scars (those that looks like small craters and pitted holes on the skin) cannot heal by themselves 2. The regenerative ability of our skin reduces with age 3.

What is the best treatment for acne scars if I still have acne outbreaks? (photo)

From the photos it seems to me your acne breakouts are still quite active, and I will not advise treating your acne scars before addressing your acne breakouts. This is because: some of the acne scarring treatments can irritate the skin and even cause breakouts themselves. if acne is undertreated, newly formed acne lesions can develop into new acne scars Isotretinoin is an effective treatment for cystic acne. But for it to work well, it has to be administered at the correct dosage and optimal duration.

What is the best treatment for fine lines under the eyes? (photo)

I understand you have a few concerns. I shall answer your concern about fine lines first. Firstly, allow me to clarify a few misconceptions you may have. Fine lines on the under-eye region are usually brought upon by: 1. UV damage caused by sun exposure 2. Loss of intrinsic collagen through aging 3. Static wrinkling of the skin as a result of repeated dynamic wrinkle formation From the picture, you do have a significant line at the under-eye regions for both eyes.

How can I get the best results after a laser ablative treatment for acne scars?

Congratulations on getting your ablative laser treatment for your acne scars. By following the post care instructions provided your laser doctor, you should be able to make a speedy recovery. When an ablative laser is applied, a proportion of the epidermis (uppermost surface of the skin) and parts of the dermis are removed temporarily. During this time, the skin barrier is considered impaired, and is more vulnerable to infection and effects of UV exposure.

Why does laser treatment work if it damages the skin and collagen, similar to smoking?

Do not fret that you felt no improvement after laser treatments. Medical lasers do work - it is just that the right lasers have to be selected for the skin condition that the patient want to address. There may be some misconceptions about the topic of skin, collagen and breakdown. To answer your question broadly, the way smoking damages the skin is very different from the way lasers or other energy devices ‘damage’ the skin.
