Can fillers be done on the upper eyelid, and what are the risks?

Yes, in short, fillers can be performed for upper eyelid hollowness and eyebrow lift. Besides the general risks of swelling and bruising with fillers, filler placement in this area requires careful placement in the appropriate plane to prevent the small but serious risk of skin necrosis and blindness because of blood vessels that lead to the back of the eye in this area. Using the right type of fillers and performed appropriately, there can be a significant and natural aesthetic result for upper eyelid hollowness. Do consult an experienced doctor for treatment. How to fix overfilled fillers at my cheeks?

In such a scenario, may I suggest a more prudent approach of combining gentle massage and radiofrequency/lasers to even out the more "overfilled" side. In my opinion, hyaluronidase injection should only be used if the above conservative approaches fail, because the amount of filler degradation on the "overfilled" side cannot be accurately predicted with hyaluronidase. I would recommend that you see an experienced Aesthetic Dr to carry out the above procedures. Hope this helps!

What anti-aging treatments are recommended for a 40 year old lady who has never undergone any aesthetic procedures before?

There are a multitude of aesthetic treatments available. Being new to aesthetic treatments, I would recommend treating your complexion issues first as this usually involves aesthetic procedures that are least invasive (medical-grade creams & lasers) and can have the most significant improvement in your overall appearance. This is followed by then addressing volume loss and skin laxity concerns with treatment options that my colleagues have mentioned. Is it normal for acne scars to get wider but shallower after acne scar treatment, such as TCA CROSS?

TCA cross especially at higher concentrations of 70-100% when used to treat ice-prick scars can possibly lead to slight "widening" of the scars. Depending on when the treatments were performed, the appearance of the scars may take up to 2-3 months to improve as collagen remodelling takes place. Beyond this period of time, you may consider injectables such as Rejuran (PDRN) or collagen-stimulators e. g Radiesse and Ellanse treatment on targetted areas, to further improve the appearance of your widen scars. How to fix under eye and tear trough hollowing?

With age, there is resorption of bone and decrease in soft tissue density around the eyes resulting in tear trough hollowness. In my practice, I use customised low-viscosity fillers depending on severity and extent of tear trough hollowness. Injecting fillers at the correct depth will yield optimum natural results with no to low downtime (swelling, bruising). Hope this helps!

Sorry to hear about your situation. I agree with both Dr Justin and Dr Winston. From the pictures you have provided, you have mainly Atrophic scars: Box and Peel-rolling types. I would advice a combination of: 1) Subscision + Collagen-stimulating fillers such as Radiesse/ Ellanse OR Rejuran S (high-viscosity PDRN specifically for acne scar treatment) to release the fibrosis causing the tethered appearance + collagen stimulation respectively.

Will I be left with worse volume loss after HA fillers are reabsorbed?

Upon absorption of the hyaluronic acid component of fillers by the body, you will not experience worse volume loss. In fact, research by filler companies such as Allergan have shown that repeated use of fillers lead to collagen stimulation. In practice, what many aesthetic medical providers have experienced is that with repeated filler injections, less amount of product is required, and/ or treatment interval can be prolonged.

Are there any effective alternatives to lasers for treating melasma?

Safe and effective alternatives to lasers for Melasma treatment include: 1) Medical-grade bleaching creams such as: Hydroquinone 4%, Triluma (Fluocinolone acetonide 0. 01%, Tretinoin 0. 05%, Hydroquinone 4%) Azelaic acid, Kojic acid etc. Various bleaching creams have their pros and cons, some being more effective but may cause more skin irritation than others. It is important to follow the customised treatment regime prescribed by your doctor, depending on your type of skin and type/ severity of Melasma. 2) Chemical Peels. Chemical peels such as Glycolic acid can help to lighten Melasma.

How effective are lasers for permanently getting rid of moles?

The first step is to rule out the possibility of any malignancy. If from history and physical examination the mole is likely benign, it can be removed either via an ablative laser or excision biopsy depending on size (main aim is to reduce risk of scarring and recurrence). Either method can be done as a quick office lunch time procedure. If the mole benign, risk of recurrence is low. Hope this helps!

What are alternative treatments to Botox and HIFU for saggy cheeks? (photo)

From a limited lower face picture that you have provided, you would benefit from liquid facelift (fillers injected at a few precise anatomical locations to give you an instantaneous & lasting lift with no to low downtime). For more lifting you may consider or combine with threadlift (options include PDO, PCL & PLLA). However, threadlift usually has a slightly longer downtime compared to liquid facelift. Removal/ prevention of facial blackheads/comedones requires simple and regular maintenance to achieve good constant results. I always advice my patients to have: 1) A proper skin care routine, in order of: (cleanse> tone> retinoid [only at night]> moisturise> sunscreen) 2) Clinic based procedures: Customised acid peels & lasers The above can be done in a cost-effective manner. What is the best method to correct an atrophic scar on my forehead? (photo)

Sorry to hear about the scar! It must have been a traumatic experience. For most optimal results, I would advise a combination of subcision + filler/Rejuran & resurfacing laser. Whether an atrophic scar would benefit from subcision + filler/Rejuran would depend entirely on clinical examination. If suitable, my recommendation would be subcision and Rejuran instead of filler for your scar over the glabella area. It is important to note that there are blood vessels from the eyes that supply this area which if compressed/blocked by fillers can lead to blindness.

What is the best treatment for fine lines under the eyes? (photo)

Hi, From a static picture that you have provided alone, I would recommend a combination of 2 types of fillers +/- botulinum toxin under the eyes for the most dramatic results. No swelling and minimal chance of bruising can be achieved with experienced injectors. That being said, it is better to examine you in person as the way your undereye looks when smiling and expressing yourself is important in determine treatment approach and outcome. Hope this helps!

Can I still get HA nose fillers after a rhinoplasty?

As Dr Justin has pointed out, yes you can have HA nose fillers after a surgical rhinoplasty. In fact, one of the indications for fillers is for further improvement/ refinement post surgical rhinoplasty, which I have performed for several patients in my practice.
